Palbociclib is an oral selective inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6), which regulate progression from the G1 to S phase of the cell cycle.

By blocking CDK4/6 activity, it prevents phosphorylation of the retinoblastoma (Rb) protein, leading to cell cycle arrest in hormone receptor–positive breast cancer cells.

It is primarily used in combination with endocrine therapy for HR-positive, HER2-negative advanced or metastatic breast cancer.

Common pharmacologic adverse effects include neutropenia, leukopenia, fatigue, and increased risk of infections.
